Best for: Patients with mild to moderate skin laxity on the face who want a well-documented, no-downtime option. Thermage has extensive clinical research behind it.

Limitations: Surface delivery means less precise energy placement than devices that bypass the epidermis. Results can be variable between patients. Treatment requires significant skill to produce consistent results.

Morpheus8

Morpheus8 is a fractional RF microneedling device: it uses an array of insulated microneedles to penetrate the skin to a programmable depth, then delivers radiofrequency energy at the needle tips. This subdermal delivery places the energy precisely in the target tissue rather than relying on it to travel through the epidermis.

Because it uses needles, Morpheus8 creates controlled injury to the skin surface as well as delivering RF energy beneath it. This dual mechanism (surface remodeling from the needles plus deeper collagen stimulation from the RF) can produce strong results, but it also means meaningful downtime: pinpoint bleeding, redness, and several days of visible skin disruption are typical.

Best for: Patients willing to accept several days of downtime for more aggressive remodeling. Particularly effective for patients with both surface texture concerns and deeper laxity who want to address both simultaneously. Popular for the lower face, neck, and body.

Limitations: Recovery period of three to seven days. Not appropriate for patients who can’t take time away from work or social obligations. More aggressive than Attiva RF in both results and downtime.

Sofwave

Sofwave is a newer device using synchronous ultrasound parallel beam technology, which is a different energy modality from RF. It delivers ultrasound energy at a very specific depth in the mid-dermis using a surface applicator, with a simultaneous cooling mechanism to protect the epidermis. No needles, no surface disruption, essentially no downtime.

Sofwave has generated strong clinical data since its launch, particularly for brow lifting and facial laxity. The depth targeting is precise and the treatment is comfortable. The main limitation is that it’s a surface applicator with a fixed depth of delivery, which means less flexibility for treating different tissue depths or body areas.

Best for: Patients wanting a comfortable, no-downtime treatment for facial laxity with a newer technology and good clinical backing. Effective for brow lifting specifically.

Limitations: Newer technology means less long-term data than older devices. Surface delivery, like Thermage, relies on energy traveling through the epidermis. Typically more limited for body applications.

Attiva RF

Attiva RF uses insulated microneedles to deliver bipolar radiofrequency energy directly into the dermis, bypassing the epidermis entirely. This subdermal delivery mechanism is what distinguishes it from surface applicator devices like Thermage and Sofwave, and what makes the energy placement more precise and controllable.

The bipolar configuration means the current travels between two electrode points within the tissue rather than through a larger circuit, which allows precise targeting of specific tissue depths.
