What’s less clearly understood by most patients is the difference between doing it alone and doing it with an enhancement, and between the two main enhancement options: PRP (platelet-rich plasma) and PDRN (polydeoxyribonucleotide).
Both are used at FACE/FIT and both produce meaningfully better outcomes than microneedling alone for the right patient. They work through different mechanisms and are appropriate for different goals. This post explains both clearly so you can come into your consultation with enough context to have a real conversation about what makes sense for you.

Why Enhancements Make a Meaningful Difference
Microneedling creates micro-channels in the dermis through controlled injury, triggering a collagen synthesis response. The channels created during treatment are open for a brief window immediately following the procedure. This window is exactly when the skin is most receptive to topically applied bioactive compounds, because the channels allow penetration well beyond what normal topical application achieves.
Applying a serum with active growth factors or regenerative compounds during this window is categorically different from applying the same compound to intact skin. The delivery mechanism is fundamentally different. This is why the enhancement choices matter and why they produce different outcomes than the same compounds taken orally or applied topically in a normal skincare routine.
Microneedling Without Enhancement
Microneedling alone produces real and meaningful results. Collagen remodeling triggered by the micro-injury response improves texture, softens scarring, and firms the skin progressively over a treatment series. For many patients, this is sufficient.
Standalone microneedling is appropriate for patients with mild to moderate concerns, patients doing maintenance treatments after a previous series with enhancement, patients for whom PRP or PDRN is contraindicated, and patients working within a tighter budget who want to prioritize the number of sessions over adding enhancements.
The results are genuine. The enhancement options simply extend the response and address specific concerns more comprehensively than the mechanical collagen stimulus alone.
PRP: What It Is and How It Works
PRP stands for platelet-rich plasma. It’s derived from your own blood: a small sample is drawn at your appointment, placed in a centrifuge that separates the components by density, and the resulting concentrated plasma, rich in platelets and the growth factors they contain, is applied topically during your microneedling session.
Platelets are the blood components involved in wound healing and tissue repair. They release a specific array of growth factors including platelet-derived growth factor (PDGF), transforming growth factor (TGF), vascular endothelial growth factor (VEGF), and epidermal growth factor (EGF), among others. These signals accelerate the healing and remodeling response that microneedling initiates.
When applied through open micro-channels, these growth factors reach the dermis directly and amplify the collagen synthesis signal that the micro-injury triggers on its own. The result is faster healing, more significant collagen response, and improvements in skin quality that exceed what microneedling alone produces.
PRP is particularly effective for:
Acne scarring, where the growth factor stimulus produces more significant collagen remodeling in the scar tissue. Overall skin quality improvement, where the combined mechanical and biological stimulus produces the most comprehensive response. Scalp microneedling for hair rejuvenation, where PRP’s effect on hair follicle activity is well-documented in clinical literature and significantly enhances the outcome compared to scalp microneedling alone.
Practical considerations: PRP requires a blood draw at your appointment, which adds a few minutes to the session and is a mild additional step most patients don’t find significant. The plasma is processed immediately and used in the same session, so there’s no stored material.
PDRN: What It Is and How It Works
PDRN stands for polydeoxyribonucleotide. It’s a regenerative compound derived from salmon DNA, specifically the DNA fragments isolated from salmon sperm, which are structurally similar to human DNA and have a well-established safety and biocompatibility profile. PDRN activates the A2A adenosine receptor, triggering a tissue repair and regeneration pathway that is distinct from the growth factor mechanism of PRP.
The A2A receptor pathway stimulates collagen synthesis, reduces inflammatory signaling, promotes angiogenesis (new blood vessel formation supporting tissue healing), and supports cellular proliferation in the dermis. PDRN has been used in wound care and regenerative medicine for decades, and its application in aesthetic microneedling reflects that established clinical track record.
When applied through micro-channels during microneedling, PDRN penetrates to the dermis and activates this regenerative pathway directly in the tissue being remodeled.
PDRN is particularly effective for:
Skin quality improvement, hydration, and the overall regenerative response. Patients with sensitive or compromised skin who want a regenerative stimulus without the pro-inflammatory component that PRP’s growth factors can produce in reactive skin. Patients who prefer not to have a blood draw. Skin that needs support beyond what collagen remodeling alone addresses, including skin that’s been depleted by UV damage, chronic stress, or the effects of systemic illness.
Practical considerations: PDRN is a prepared compound added to the treatment at the time of microneedling. There’s no blood draw. It’s appropriate for virtually all patients who are candidates for microneedling.
PRP vs PDRN: The Direct Comparison
Both enhance microneedling outcomes beyond the standalone treatment. They work through different mechanisms and produce overlapping but not identical results.
PRP’s growth factor combination produces a strong pro-regenerative signal that’s particularly effective for collagen remodeling in established scarring and for hair follicle activation. The autologous source means it’s derived entirely from your own biology. The blood draw is a minor additional step.
PDRN’s A2A receptor activation produces a tissue repair and hydration response that’s particularly effective for overall skin quality, compromised or sensitive skin, and patients who want a strong regenerative stimulus without a blood draw.
For patients with significant acne scarring or who are pursuing scalp microneedling for hair loss, PRP is typically our first recommendation. For patients focused on skin quality, hydration, texture, and overall cellular renewal, PDRN is often as effective or more so. Some patients benefit from alternating both across a treatment series.
The decision is made at your consultation based on your specific skin, your concerns, and your goals. There isn’t a universally correct answer.

One of the most significant and underutilized applications of microneedling with PRP is scalp treatment for hair thinning and loss. FACE/FIT offers Hair Rejuvenation scalp microneedling, which combines the mechanical follicle stimulation of scalp microneedling with PRP’s well-documented effects on hair follicle biology.
PRP applied to the scalp during microneedling delivers growth factors directly to the follicle environment. The combination of increased blood supply from the micro-injury response and the PDGF and VEGF in PRP produces a synergistic effect on follicle activity that has been demonstrated in clinical research for androgenetic alopecia and other forms of diffuse hair thinning.
This treatment is particularly relevant for patients experiencing peri or post-menopausal hair thinning, hair loss following significant weight loss on GLP-1 medications, stress-related shedding, and the gradual diffuse thinning that affects both men and women with age. It doesn’t reverse advanced alopecia or treat scarring alopecia, but for the appropriate patient profile it produces meaningful and visible improvement in hair density over a treatment series.
What to Expect During a Microneedling Session With Enhancement
The session proceeds similarly whether you’re adding PRP, PDRN, or doing standalone microneedling. The differences are procedural.
For PRP: a blood draw happens at the start of your appointment, typically from the arm. The sample takes approximately 10 minutes to process in the centrifuge while your provider completes the pre-treatment preparation and numbing. The PRP is then applied to the treatment area during the microneedling pass.
For PDRN: the compound is prepared and applied topically during the microneedling pass without any blood draw.
In both cases, the enhancement is introduced into the micro-channels while they’re freshly created, which is the window of optimal absorption. The total session time is approximately 45 to 75 minutes depending on areas treated.
Post-treatment care is the same regardless of which enhancement is used: gentle cleanser, fragrance-free moisturizer, mineral SPF, and no active ingredients for several days. Avoid strenuous exercise and heat exposure for 24 hours.
Frequently Asked Questions
Is PRP or PDRN more effective?
They’re more effective at different things. PRP is typically the stronger choice for acne scarring and scalp hair rejuvenation. PDRN often produces better results for overall skin quality, hydration, and sensitive skin. We make the recommendation based on your specific goals at your consultation.
Can PRP and PDRN be used together?
Yes. Some treatment plans alternate PRP and PDRN sessions across a series to leverage both mechanisms. We’ll discuss whether a combined approach makes sense for your goals.
Does PRP hurt?
The blood draw is a standard venipuncture, comparable to a routine blood test. The application of PRP during microneedling doesn’t add discomfort beyond the microneedling itself.
How many sessions do I need?
For general skin quality improvement, three sessions spaced four to six weeks apart is a standard starting recommendation. For acne scarring, four to six sessions is typically needed for significant improvement. For scalp hair rejuvenation, we typically recommend a minimum of three sessions and assess from there.
Is there a reason to choose standalone microneedling over an enhanced version?
For maintenance treatments after a completed initial series, or for patients with very mild concerns, standalone microneedling is appropriate and cost-effective. For patients doing their first series or targeting significant scarring or skin quality concerns, an enhancement typically produces meaningfully better outcomes for the additional investment.
How soon will I see results?
With enhancement, early improvement in skin texture and quality is often noticeable four to six weeks after the first session. Collagen remodeling continues for up to six months after the final session, with results improving progressively throughout.
