FACE/FIT provides aesthetic, wellness, and medical spa services, which may include but are not limited to consultations, neurotoxin treatments, dermal fillers, skin treatments, facials, chemical peels, microneedling, laser and device-based treatments, IV hydration therapy, medical-grade skincare, wellness treatments, and related services.
All treatments are subject to provider evaluation, medical appropriateness, patient eligibility, informed consent, and availability. Not all services are appropriate for every patient. FACE/FIT reserves the right to decline, delay, modify, or discontinue any service when clinically appropriate or necessary for patient safety.

2. No Provider-Patient Relationship Through Website Use
Use of our website, submission of an online form, chatbot interaction, email communication, SMS message, phone inquiry, or appointment request does not, by itself, create a provider-patient relationship.
A provider-patient relationship is established only after you have completed applicable intake and consent forms, been evaluated by an authorized FACE/FIT provider, and have been accepted for care or treatment.
If you are experiencing a medical emergency, call 911 or seek immediate emergency medical care. Do not use our website, forms, chatbot, SMS, email, or social media messaging for urgent or emergency medical needs.
3. Consultations and Treatment Eligibility
FACE/FIT may offer in-person or virtual consultations. Consultations are intended to help us understand your goals, review relevant health history, discuss possible treatment options, and determine whether treatment may be appropriate.
You agree to provide accurate, complete, and current information during consultations, intake forms, consent forms, medical history questionnaires, and any other communications with FACE/FIT. Aesthetic and wellness outcomes vary from person to person. Results may depend on age, anatomy, skin condition, lifestyle, health history, treatment plan, product selection, aftercare compliance, metabolism, and other individual factors.
FACE/FIT does not guarantee any specific result, outcome, duration, improvement, or appearance from any service, treatment, product, or recommendation. Before-and-after photos, testimonials, social media content, and website images are examples only and do not guarantee that you will achieve the same or similar results.
5. Appointment Booking, Cancellations, and No-Shows
Appointments may be requested online, by phone, by text, or through other booking methods we make available. Appointment requests are not confirmed until accepted by FACE/FIT or our booking platform.
FACE/FIT may require a deposit, card on file, consultation fee, or prepayment to reserve an appointment. By booking an appointment, you agree to comply with our cancellation, rescheduling, late arrival, and no-show policies as provided at the time of booking or in our office policies.
FACE/FIT reserves the right to charge cancellation or no-show fees, require deposits for future bookings, or decline future appointments for repeated late cancellations, no-shows, or policy violations.
6. Pricing, Promotions, and Payment
Prices listed on our website are provided for general reference and may change at any time without notice. Final pricing may vary based on consultation findings, treatment area, product amount, number of sessions, provider recommendation, membership status, promotions, or individualized treatment planning.
Payment is due at the time of service unless otherwise agreed in writing. We may accept credit cards, debit cards, financing options, gift cards, membership credits, or other payment methods at our discretion.
Promotions, discounts, packages, memberships, and special offers may be subject to separate terms, expiration dates, eligibility requirements, and limitations. Promotions may not be combined unless expressly stated.
All sales of services, packages, memberships, gift cards, deposits, and skincare products may be subject to FACE/FIT’s refund, exchange, and cancellation policies.
7. Memberships, Packages, and Loyalty Programs
FACE/FIT may offer memberships, packages, loyalty programs, recurring billing options, promotional credits, or similar programs. Memberships and packages may be subject to separate written terms, including billing frequency, minimum commitment period, cancellation requirements, rollover rules, expiration dates, service exclusions, and refund limitations.
By enrolling in a membership or recurring payment plan, you authorize FACE/FIT or its payment processor to charge your selected payment method according to the membership terms presented at enrollment.
Membership credits, discounts, benefits, and promotional perks have no cash value unless otherwise required by law. FACE/FIT reserves the right to modify, suspend, or discontinue memberships, packages, loyalty programs, or promotional benefits at any time, subject to applicable law and any written agreement in place.
8. Products and Skincare Recommendations
FACE/FIT may sell or recommend skincare products, medical-grade skincare, supplements, or related items. Product recommendations are based on the information available at the time and may not be appropriate for everyone.
You agree to review product instructions, warnings, ingredients, and usage guidelines before use. Discontinue use and contact a qualified healthcare provider if you experience irritation, allergic reaction, or unexpected side effects.
Unless otherwise stated, opened skincare products, used products, and special-order items may not be eligible for refund or exchange.
